Additionally, during your consultation, we may ask for medication details. This information is crucial in the unlikely event of a medical emergency during a session, enabling us to provide accurate information to emergency services and ensure appropriate treatment planning.
Session Notes:
We maintain brief and anonymous session notes securely to aid in the continuity and effectiveness of our counseling sessions.
Data Sharing:
Your privacy is paramount, and we assure you that your data will not be sold or used for marketing purposes.
However, in the event of a court subpoena or if there are concerns about harm or risk of harm, we may be obligated to share information. Rest assured, we take every precaution to protect your confidentiality, and any disclosures will be made with careful consideration and in accordance with legal and ethical guidelines.
Data Storage:
We operate a paperless practice, storing all data digitally and ensuring it is password protected for security.
Data Retention:
We will retain the privacy statement with your signature, session notes, client record sheets, and relevant emails (if deemed clinically necessary) for up to 1 year after our work concludes.
Personal data such as phone numbers and email addresses will be securely destroyed within 2 months after our last meeting.
